Soda and fruit juice are 'biggest culprits in dental erosion'

Soft drinks are the most significant factor in severity of dental erosion, according to a new study published in the Journal of Public Health Dentistry.

The most severe cases of dental erosion in the study were among people who regularly drank sugary soft drinks and fruit juices.
Dental erosion is when enamel - the hard, protective coating of the tooth - is worn away by exposure to acid. The erosion of the enamel can result in pain - particularly when consuming hot or cold food - as it leaves the sensitive dentine area of the tooth exposed.
The enamel on the tooth becomes softer and loses mineral content when we eat or drink anything acidic. However, this acidity is cancelled out by saliva, which slowly restores the natural balance within the mouth. But if the mouth is not given enough time to repair itself - because these acid attacks are happening too often - the surface of the teeth is worn away.
Anything with a pH value (the measure of acidity) lower than 5.5 can damage the teeth. Diet and regular sodas, carbonated drinks, flavored fizzy waters, sports drinks, fruit and fruit juices are all known to be harmful to teeth if they are consumed too often.

Study finds that a 'substantial proportion' of adults have dental erosion

The study finds that a substantial proportion of adults show some evidence of dental erosion, with the most severe cases being among people who drink sugary soft drinks and fruit juices.
Examining 3,773 participants, the researchers found 79% had evidence of dental erosion, 64% had mild tooth wear, 10% had moderate tooth wear and 5% displayed signs of severe tooth wear. The participants in the study with moderate and severe tooth wear consumed more soft drinks and fruit juices each day than the other groups.
Among participants with lower levels of tooth wear, the researchers found that milk was a more popular drink than soda or fruit juice.
Men were also found to be at twice the risk for dental erosion as women, and tooth wear became more severe with age among the participants.
Commenting on the study, Dr. Nigel Carter OBE, chief executive of the British Dental Health Foundation, says that while fruit juice may be a nutritious drink, the high concentrations of sugar and acid can lead to severe dental damage if these drinks are consumed often each day.
"Water and milk are the best choices by far, not only for the good of our oral health but our overall health too," says Dr. Carter. "Remember, it is how often we have sugary foods and drinks that causes the problem so it is important that we try and reduce the frequency of consumption."
He adds:
"Dental erosion does not always need to be treated. With regular check-ups and advice your dental team can prevent the problem getting any worse and the erosion going any further. The more severe cases of tooth wear can often result in invasive and costly treatment so it is important that we keep to a good oral hygiene routine to make sure these future problems do not arise."
Many sodas and fruit juices contain at least six teaspoons of sugar, and as they often come in portions that are larger than recommended, they can lead to tooth decay as well as dental erosion.

Oral Health: The Mouth-Body Connection

Years ago, a physician who suspected heart disease would probably not refer the patient to a gum specialist. The same went for diabetes,pregnancy, or just about any other medical condition. Times have changed. The past 5 to 10 years have seen ballooning interest in possible links between mouth health and body health.
"Physicians are taking a more holistic approach to their patients’ overall health," says Sally Cram, DDS, PC, consumer advisor for the American Dental Association. And for good reason. In one recent study, people with serious gum disease were 40% more likely to have a chronic condition on top of it.
In this article, WebMD answers two questions about the mouth-body connection. Why can the health of your mouth affect your whole body? And why are simple habits like daily brushing and flossing more important than you might think?

Your Mouth, the Gateway to Your Body

To understand how the mouth can affect the body, it helps to understand what can go wrong in the first place. Bacteria that builds up on teeth make gums prone to infection. The immune system moves in to attack the infection and the gums become inflamed. The inflammation continues unless the infection is brought under control.
Over time, inflammation and the chemicals it releases eat away at the gums and bone structure that hold teeth in place. The result is severegum disease, known as periodontitis. Inflammation can also cause problems in the rest of the body.

Oral Health and Diabetes

The working relationship between diabetes and periodontitis may be the strongest of all the connections between the mouth and body. Inflammation that starts in the mouth seems to weaken the body’s ability to control blood sugar. People with diabetes have trouble processing sugar because of a lack of insulin, the hormone that converts sugar into energy.
"Periodontal disease further complicates diabetes because the inflammation impairs the body’s ability to utilize insulin," says Pamela McClain, DDS, president of the American Academy of Periodontology. To further complicate matters, diabetes and periodontitis have a two-way relationship. High blood sugar provides ideal conditions for infection to grow, including gum infections. Fortunately you can use the gum disease-diabetes relationship to your favor: managing one can help bring the other under control.

Understanding your numbers: The dental hygienist's role in business success

Dental practices, as businesses, are comprised of three main departments: the business desk, the restorative department, and the hygiene department. In this article, we will focus on the hygiene department as a major factor in dental practice success. We will examine several industry benchmarks and discuss ways to improve hygiene department efficiency and productivity.

When to hire a hygienist

The most valuable commodity any dentist possesses is his or her time. How the dentist uses time is an important consideration in business success. When a dentist chooses to perform procedures that could be delegated to a qualified auxiliary, the dentist's time is being used inefficiently.

There are only two reasons that a dentist should be doing hygiene procedures. First, if the practice is a start-up practice with very few patients, the dentist may need to perform hygiene procedures just to stay busy. It takes from 800 to 1,000 active patients to keep one full-time hygienist busy. So, in the start-up stage, the practice may need a hygienist only part-time until the practice grows to sufficient size to employ a full-time hygienist. If a dentist spends even 25% of his or her time performing hygiene procedures, that means that 25% of the dentist's time could have been spent performing more productive restorative procedures.
The other reason for not employing a hygienist is if the practice is located in an area where there are no qualified hygienists. That is rarely the case today, and in most states there is an oversaturation of the market for hygiene positions. Many states report that hygiene graduates are having great difficulty finding full-time work. As an example, a dentist in North Carolina ran an ad on craigslist for a hygienist and received 30 resumes in two days.

Hygiene department industry benchmarks

Benchmarks are standards in an industry that are used as a measurement of achievement or excellence. Here are some typical hygiene department benchmarks:
1. The hygiene department will contribute 25% to 35% of gross practice production.
2. Seventy-five percent of restorative needs are discovered in hygiene patient visits.
3. One-third of hygiene codes will be periodontal codes.
4. Downtime should be no higher than 5% to 8% of available time.
5. Hygienists should produce about three times their wages.
6. Hygienists are optimally positioned to promote restorative and elective procedures.
While benchmarks are meaningful, it is important to recognize that they are not always applicable. Let's look at how some of these benchmarks might be skewed.
The first benchmark assumes that the doctor/hygienist ratio is one doctor per one hygienist, which is not always the case. In some situations, a solo doctor may employ multiple hygienists, and the hygiene department may contribute well above 35% of gross production. When the hygiene department contributes an inordinately high percentage of gross production, this usually is a sign of a weak restorative department. Another factor where hygiene department production may not follow the benchmark is when the practice has a niche in the cosmetic dentistry market (5% or fewer practices nationwide) and the doctor performs a predominance of high-dollar procedures. In this case, the hygiene department may contribute less than 25% of the total gross production.
The third benchmark is related to periodontal coding. While we like to see a healthy number of periodontal codes (D4341, D4342, D4910) in the procedure mix, this metric can be skewed in two ways. First, if there are not at least 20 to 25 new patients per month per doctor, the practice may not see as many periodontal codes in the mix. New patients offer the best opportunity for periodontal treatment in many practices, but a lack of new patients causes the practice to "stagnate." The majority of established patients may be well maintained and free of periodontal problems. Second, periodontal disease may be underdiagnosed in some practices due to lack of an established periodontal program and/or lack of time and assistance for hygienists needing to perform periodontal charting. When hygienists are pressed for time, the most omitted procedure is periodontal charting, simply because it is so time-consuming to chart and record solo. Hygienists are more likely to periodontal chart when the barriers to charting are removed by making assistance available or investing in a voice system.

The impact of downtime

Of all the systems in a dental practice, there is nothing more important than the schedule. Yet, it is common to see one to two open appointments in hygiene each day. If the average cost of a hygiene visit is $150, two openings per day in hygiene cost $60,000 per year in lost productivity. Downtime in the hygiene schedule not only means a loss of productivity, but there is also a corresponding rise in overhead costs, and the restorative schedule will be negatively affected by undiagnosed patient needs. Downtime is the greatest profitability killer in the practice. Let's examine how to measure downtime (also see Table 1 for a sample downtime analysis):
1. Tally all the available units of time that the hygienist is available to see patients in one month. If the practice is on a 15-minute time increment, there will be four time units in an hour. An 8-5 day with an hour for lunch would mean 32 time units per day. If the office is on a 10-minute time increment, there are six units in an hour. An 8-5 day would have 48 time units.
2. Tally all the open time units in the schedule. Do not count lunch as open time. Also, do not count time that has been blocked off for training or staff meetings.
3. Divide the available unit total into the open time unit total (big number into the small number). Move the decimal two places to the right. This is the downtime percentage. Round up anything that is .5 or higher.
As you can see from this example, downtime is very costly and can destroy a practice's profitability. In order to correct the situation, practices need to track and analyze downtime. If demand for services has fallen, the doctor can either reduce available hours or engage in some effective marketing to increase the number of new patients. It would be advantageous for the practice to concentrate on reactivating unscheduled patients and improve the show rate through better communication. Is the business desk understaffed? Does the business assistant need some training in systems or better communication? Does he or she understand the importance of proper scheduling? Is he or she motivated to improve?

The salary/production ratio

The fifth benchmark is about the hygienist's salary/production ratio. The industry standard is that a hygienist should produce around three times his or her wages. One caveat that can skew this benchmark is when hygiene fees are artificially lower than normal. Some doctors raise fees on restorative procedures but keep hygiene fees low in an effort to promote an illusion of low fees to patients. In all fairness, hygiene fees should be raised when restorative fees are raised.
Let's compare two hygienists. Table 2 shows how many of each procedure code was performed over a three-month period. It also shows each hygienist's wages, then a computation of the salary/production ratio, which is derived by dividing production into wages and moving the decimal two places to the right.
Table 2 shows Katherine has a 47% salary/production ratio, which is well above the benchmark of 33%. The high ratio could be a function of low production because of excessive downtime, but what stands out in the analysis is the lack of periodontal procedures. Periodontal therapy is the most productive procedure for hygienists. Notice that there were only two quads of periodontal debridement, and there were no site-specific or periodontal maintenance procedures. Most likely, periodontal problems are being underdiagnosed in this practice. It seems unreasonable to believe that there was not one pocket in a total of 228 patients seen.
Christy's analysis is more encouraging. There was a total of 83 periodontal codes (D4341, D4342, D4910), and it appears Christy is engaged in identifying periodontal problems and ushering patients into definitive treatment. Periodontal treatment is 36% of codes. Her salary/production ratio is under the benchmark, and an increase in pay is reasonable and appropriate.

A hygienist's intangible value

Dental hygienists bring more to the dental business than raw production numbers. Equally important is a hygienist's understanding of restorative and elective procedures and identification of patients that would benefit from the doctor's skills. Hygienists are in the best position of all the dental auxiliaries to promote restorative procedures. But in order to promote something, the hygienist must have a clear understanding of those procedures and be able to broach the subject of elective dentistry in a way that piques patient interest without overselling. They need to feel confidence in the doctor's ability to deliver excellent results in order to promote advanced restorative and cosmetic procedures.
Doctors should take their whole team to courses aimed at elective procedures so staff members can learn along with the doctor. Hygienists should be invited to observe certain elective procedures while being performed so they can better understand the process and learn how procedures are done.
Hygienists should track the amount of elective restorative dentistry that is discussed and scheduled with patients. This data can be presented at the monthly staff meeting and may serve the hygienist well in future salary negotiations.
There are several good ways to improve hygiene department productivity.
1. Improve scheduling. Don't automatically give every patient one hour, but rather schedule for needs. Be proactive in deciding which procedures will happen on the next visit, and schedule appropriately.
2. Decrease downtime with effective communication skills. Identify chronic broken appointment offenders. Never leave reminder messages for new patients or people with a history of disappointment, but speak with them directly, even if they have to be called after hours. Make sure every patient who misses an appointment is contacted. Reactivate patients who are past due.
3. Improve the procedure mix by identifying periodontal problems and ushering the patient into definitive care. Have your "periodontal radar" engaged at all times.
4. Offer adjuncts such as power brushes, water lavage tools, and intrapocket therapies when appropriate.
5. Conduct full-mouth, six-point comprehensive periodontal charting/recording on all adult patients once per year.

6. Implement assisted hygiene. The assisted model is undoubtedly the best way to increase hygiene department efficiency and productivity without compromising quality.
Knowing that the hygiene department typically drives the rest of the practice, the savvy 21st century dental hygienist understands two important facts: (1) hygienists must provide excellent patient care, and (2) hygienists should be interested in the specific metrics associated with their work. Specifically, they should understand what percentage of the total gross production they provide, percentage of periodontal procedures, salary/production ratio, downtime percentage, and monetary amount of elective dentistry presented and scheduled. Hygienists should also be innovative problem-solvers when number-crunching reveals areas of concern. RDH
